The Effect of Acupuncture on tinnitus handicap inventory (THI) score in tinnitus patients

ABSTRACT

Background: Tinnitus is the sensation of sound in the absence of stimulation, either electrical or mechano-acoustic, from the external environment. It usually occurs for at least five minutes and more than once a week. Treatment of tinnitus has been done empirically. The aim of this study was to determine the success rate of acupuncture in reducing tinnitus disability inventory (THI) scores in patients with tinnitus. Ten sessions of acupuncture therapy were performed on 16 patients and the mean THI scores were evaluated before and after therapy. The success rate of the therapy was 56.3%. The mean THI score before therapy was 30 which decreased to 21.63 after therapy (p<0.05). In summary, acupuncture decreased THI scores in patients with tinnitus.�

Case: Male and female patients aged 18-70 years, who had been diagnosed with sensorineural disorders and tinnitus complaints at the ENT polyclinic of Cipto Mangunkusumo Hospital with THI score ≥18. Subjects were not undergoing any other therapy for tinnitus and had no contraindications for acupuncture therapy (severe hypertension, diabetes mellitus, fever, or pacemaker. Manual acupuncture was performed on TE17 Yifeng, SI19 Tinggong and Jiao Sun Fa scalp hearing lines for 20 minutes. Electroacupuncture was performed at points LI4 Hegu, ST36 Zusanli with a frequency of 2 Hz with dense disperse waves.  Therapy was performed twice a week for 12 appointments and THI score was evaluated after 10 punctures.�

Results: Significant decrease in the mean score of functional THI subscale before and after acupuncture (p<0.05). A significant decrease in the mean score of the emotional THI subscale before and after acupuncture (p<0.05). A significant decrease in the mean score of catastrophic THI subscale before and after acupuncture (p<0.05). A significant decrease in the mean total THI score before and after acupuncture from 30% to 20.63% with a value of (p<0.05). The success rate of therapy with a decrease in THI score ≥10%. No hematoma was found after acupuncture and the duration of therapy was shorter.�

Conclusion: Acupuncture therapy showed a favorable effect on tinnitus patients with a success rate of 56.3%. The average THI score before therapy was 30 and after therapy was 21.63 (p<0.05). The main benefits of acupuncture therapy with its relative safety.

ABSTRAK

Latar belakang : Tinnitus adalah sensasi suara tanpa adanya rangsangan, baik elektrik maupun mekano-akustik, dari lingkungan luar. Biasanya terjadi setidaknya selama lima menit dan lebih dari sekali dalam seminggu. Pengobatan tinnitus telah dilakukan secara empiris. Tujuan dari penelitian ini adalah untuk mengetahui tingkat keberhasilan akupunktur dalam menurunkan skor inventaris cacat tinnitus (THI) pada pasien dengan tinnitus. Sepuluh sesi terapi akupunktur dilakukan pada 16 pasien dan skor THI rata-rata dievaluasi sebelum dan sesudah terapi. Tingkat keberhasilan terapi adalah 56,3%. Rata-rata skor THI sebelum terapi adalah 30 yang menurun menjadi 21,63 setelah terapi (p<0,05). Akupunktur menurunkan skor THI pada pasien dengan tinnitus.�

Kasus : Pasien laki-laki dan perempuan usia 18-70 tahun, yang telah didiagnosis gangguan sensorineural dan keluhan tinitus di poliklinik THT Rumah Sakit Cipto Mangunkusumo dengan skor THI ≥18. Subjek tidak sedang menjalani terapi lain untuk tinnitus dan tidak memiliki kontraindikasi untuk terapi akupunktur (hipertensi berat, diabetes melitus, demam, atau alat pacu jantung. Akupunktur manual dilakukan pada titik TE17 Yifeng, SI19 Tinggong dan garis pendengaran kulit kepala Jiao Sun Fa selama 20 menit. Elektroakupunktur dilakukan pada titik  LI4 Hegu, ST36 Zusanli dengan frekuensi 2 Hz dengan gelombang dense disperse.  Terapi dilakukan dua kali seminggu selama 12 kali pertemuan dan skor THI dievaluasi setelah 10 kali penusukan.�

Hasil : Penurunan signifikan pada rata-rata skor subskala THI fungsional sebelum dan sesudah akupunktur (p<0,05). Penurunan signifikan pada rata-rata skor subskala THI emosional sebelum dan sesudah akupunktur (p<0,05). Penurunan yang signifikan pada rata-rata skor subskala THI katastropik sebelum dan sesudah akupunktur (p<0,05). Penurunan yang signifikan pada rata-rata skor total THI sebelum dan sesudah akupunktur dari 30% menjadi 21,63% dengan nilai (p<0,05). Tingkat keberhasilan terapi dengan penurunan skor THI ≥10%. Tidak ditemukan hematom setelah akupunktur dan durasi terapi lebih singkat.�

Kesimpulan : Terapi akupunktur menunjukkan efek yang menguntungkan pada pasien tinnitus dengan tingkat keberhasilan 56,3%. Rata-rata skor THI sebelum terapi adalah 30 dan setelah terapi adalah 21,63 (p<0,05). Manfaat terapi akupunktur yang utama dengan keamanannya relatif.
